This service is funded by the Medicaid Waiver. Contact your Supports Coordinator to arrange for a referral to our program. Simply request a tour. Once we have met with you and received the required referral information, we will schedule a trial visit and then make a decision. If you need assistance with this we can help. Each situation is different but typically three to four weeks.

Our doors open at 8:30 a.m. and participants may stay as late as 3:30 p.m. Program activities take place between 9:00 a.m.–3:00 p.m.

Yes! Adult participants may choose to participate in any of the daily / weekly activities offered.
Frazer Center does not provide meals. Participants can bring their own lunch and use the Center’s refrigerators and microwaves.
Our staff provides assistance with meals and eating, toilet assistance, and reminders to take self-administered medications.
